MH/ED/BD support
Provision requested
The MH/ ED/ BD support section is included in an access request when ‘MH/ ED/ BD Support’ is chosen by the author in the set up section. This section is required when the disability confirmation sheet has a mental health diagnosis (MH1, MH2 or MH3). It is also required when the provisions requested are for a behaviour support class or tutorial centre.
Note: the information required in the MH/ ED/ BD support section is reduced when a behaviour support plan is included in the risk assessment section of the access request
